Another option to report a claim is via e-mail within 24 hours of the incident (or as soon as possible):

E-mail: claims@pieinsurance.com

In the e-mail, include the following information:

  • Name of business
  • Policy Number
  • Reporting party’s contact information (name, phone, e-mail)
  • The name, phone and e-mail address of injured employee
  • Date of incident
  • Location of incident (address and/or city, state) 
  • Description of incident

In case of injury

AK Form 6100 - Employee report of injury

AK Form 6101 - Employer report of injury

Complete the AK Form 6101-Employer’s Report of Injury. This form must be completed and sent immediately, and in no case later than ten (10) days after you have knowledge that your employee has been injured, or claims to have been injured or become ill while working for you.

What is the waiting period for lost wages in Alaska:

No payment is due for the first 3 days of disability unless the disability lasts longer than 28 days. The first 3 day waiting period will be paid retroactively if the disability exceeds 28 days.
